Insights gathered through our annual Health Needs Assessments highlight emerging health concerns and areas for service improvement. Over the past year we have delivered programs and initiatives that have responded to aged care, digital health, youth, mental health and alcohol and other drug needs in our community.

Person-centred healthcare is about working with patients and listening to what consumers value central to their care journey. We always encourage our providers to take a ‘nothing about us, without us’ approach when developing services and initiatives.
This year, we commissioned initiatives that filled service gaps, improved patient experience and created inclusive and accessible environments for health care.

Listening to the needs of primary care professionals and their patients creates opportunities to design and deliver purposeful education, tools, resources and networking events. Throughout the year we collaborated with health care providers to deliver targeted staff training and educational resources, which led to enhanced capacity and capability to provide quality care in south east Melbourne.
